Allogeneic Red Blood Cell Transfusion Following the Total Hip Arthroplasty Triggers the Immunomodulatory Responses

چکیده مقاله:
Background and Aims: Total hip arthroplasty (THA) is a common orthopedic surgery. The main focus in the research of THA is immune responses in these patients following the blood transfusion. Therefore, in this study, we evaluated the immune modulation indexes such as T cell amplification, Tcell surface markers and also production of cytokines in the patients undergoing THA surgery following blood transfusion. Materials and Methods: A total of 30 patients referred to Imam Hussein Hospital for THA surgery were included in the study between May 2015 to May 2016. Transfusions of 500 ml allogeneic red blood were done during and immediately after recovery of THA operation. Along with pre- and post-operation, immune modulatory effects of transfusion, including proliferation of T lymphocyte, secretion of T cell cytokines such as interferon gamma (IFN-γ), Interleukin(IL)-4 and expression of T lymphocyte surface markers such as human leukocyte antigen D related (HLA-DR) and CD25 were evaluated. Results: Blood transfusion induced the expression of CD25 and HLA-DR markers in the THA patients. Blood transfusion also increased the secretion of IL-4 in post-operative stages in the THA patients. Results showed the increased IL-4 levels after blood transfusion. Whereas the levels of secretion in INF- γ were decreased after blood transfusion. Conclusions: Allogeneic blood transfusion in the patients with the THA induced a cascade of immune and inflammatory responses, including increasing in T cell amplification and change in the secretion of interleukins that causes a modulation of immune responses. Further investigations should be performed to verify this hypothesis.

Autologous blood transfusion in total hip arthroplasty.
PURPOSE To determine the possibility of avoiding homologous blood transfusion during total hip arthroplasty, and to clarify the problems associated with autologous blood transfusion. METHODS A total of 253 patients received autologous blood transfusion during total hip arthroplasty between April 1990 and December 2000. Patients were assessed for the volume of haemorrhage during surgery, possi...

OBJECTIVE In this study, we aimed to measure changes in red blood cell (RBC) immunity and cytokine levels after performing total hip replacement surgery. MATERIAL AND METHODS Twenty patients receiving total hip arthroplasty were investigated by measuring presurgical and postoperative RBC natural tumor erythrocyte rosette rate (NTERR), RBC C3b receptor rosette rate (RC3bRR), RBC membrane CD35,...
متن کاملAllogeneic blood transfusion and prognosis following total hip replacement: a population-based follow up study
BACKGROUND Allogeneic red blood cell transfusion is frequently used in total hip replacement surgery (THR). However, data on the prognosis of transfused patients are sparse. In this study we compared the risk of complications following THR in transfused and non-transfused patients. METHODS A population-based follow-up study was performed using data from medical databases in Denmark. We identi...

Babesiosis is a potentially life-threatening zoonotic disease that is endemic to the northeastern United States and increasing in prevalence worldwide. Transmitted by the same Ixodes tick responsible for Lyme disease, the intraerythrocytic parasite Babesia causes a wide range of clinical presentations--from asymptomatic carriage to a fulminant course with rapid deterioration. Symptoms typically...
متن کاملComplications Following Total Hip Arthroplasty
Distal deep vein thrombosis (DVT) can range from being asymptomatic, to resulting in long term valvular damage resulting in chronic venous insufficiency. Proximal propagation can result in more serious pulmonary embolism. The overall incidence of DVT, including both radiologically diagnosed asymptomatic DVT and symptomatic DVT, post THA in early studies was reported to be as high as 70% without...

We report six cases of contralateral limb involvement during total hip arthroplasty including swelling of the gluteal muscle compartments, rhabdomyolysis, myoglobinuria, and sciatic nerve palsy. The risk factors for such complications include obesity, prolonged operative time, and positioning in the lateral decubitus position. The laboratory and clinical findings are consistent with a gluteal m...
